Clinic Hours: Monday - Friday 8:00 a.m. - 4:30 p.m. 1845 W City Drive Elizabeth City, NC (757) 722-9961 PRIMARY CARE SERVICES: Primary Care - Evaluation and management of chronic and acute medical conditions; preventative health disease; health maintenance. Mental Health - Individual and group psychotherapy; psychopharmacology to treat and manage a variety of psychiatric diagnosis. Laboratory Services - Phlebotomy services will be available onsite. The Hampton VA Medical Center will perform the tests and results will be readily available to the clinician via computer. Pharmacy Services - Consultation and medication reconciliation; Lipid, Diabetes and Anticoagulation management. Social Work Services - Coordination of care; Psychosocial assessments.
