Chapel Street CBOC

Conveniently accessible by public transportation, Chapel Street Community Based Outreach Center (CBOC) is a hub for services related to homelessness, primary care, substance abuse and mental health. Breakfast and lunch are provided to homeless Veterans. The CRRC also offers on-site medical primary care and referrals to other VA specialty services as necessary. Chapel Street has an extensive network of community partners who can help with legal advice, vocational counseling, family care, spiritual care and information on benefits. They also work with the DHS to provide homeless veterans emergency housing with supportive services on site. VA Shuttle buses are available for patient transportation to VA’s NY and BK campus for referrals to other VA specialty services.
