Primary health care services include: Medication management Diabetes management Dietitian Services Patient-Family health education Patient assessment Care planning Nursing services Phlebotomy Telephone care Behavioral Health services are available on-site at the staffed CBOCs and are available by fee basis from the contracted CBOC. Services available include intake, all varieties of counseling, and medication management. Services are available either ‘live’ or via video teleconferencing.
