Evanston Primary Care Telehealth Outreach Clinic

Both primary medical and mental health services are available a the Primary Care Telehealth Outreach Clinics (PCTOC) through telehealth technology. Those Veterans in need of services not available at the PCTOC are either referred to the Sheridan VAMC, another VA facility within VISN19, or are scheduled for purchased care with in the local community. For emergency care, all CBOC/PCTOC activate the local 911 system and Veterans are taken to the local medical facility. Primary Care Telehealth Outreach Clinics are staffed with nurses. Patients will receive nursing care and other care via 21st Century technology. The global media systems allow for medical and mental health examinations and appointments to be held over a secure internet system. Clinic hours will be Monday - Wednesday 8am -4:30pm and Thursday 8am-2:30pm Closed Friday and holidays.
