The Southwest Primary Care Clinic offers the following services: Agent Cashier Enrollment Laboratory Mental Health Nutritional Medicine OEF/OIF/OND Veteran Transition Center and Care Management Team Patient Advocate Pharmacy Primary Care Release of Information Radiology Social Worker Parking, Public Transportation, and Shuttle Service Parking: There are two (2) entrances/exists to the parking lot: The main entrance is from the Southwest side of the building and one (1) from the Northwest side of the building. Shuttle Service: We also have a Shuttle Service (waiting for update on new schedule) to and from our clinics. Driving Directions General Location The new Southwest Primary Care Clinic is located on the WEST side of Buffalo., between I-215 and Warm Springs Road. Coming from East: Heading NORTH on I-215, take the Buffalo Drive exit. Make a LEFT onto Buffalo Drive. The Southwest Primary Care Clinic will be on your RIGHT. If you’ve reached Warm Springs Rd., you will see the Clinic on the corner. Coming from the West: Head SOUTH on I-215, exit onto Buffalo Drive, exit. Make a RIGHT onto Buffalo Drive. The Southwest Primary Care Clinic will be on your RIGHT. If you’ve reached Warm Springs Rd., you will see the Clinic on the corner.
