Best reached by MTA bus or car, the Staten Island Community Based Outreach Center (CBOC) that provides comprehensive primary care, psychiatric care, social work, pharmacy, pain management, optometry, podiatry, nutrition, patient education and telehealth to eligible veterans. VA Shuttle buses are available for patient transportation to VA’s Brooklyn campus for referrals to other VA specialty services. Driving Directions from the North Take the Palisades Interstate Pkwy South to exit 9W for I-87 W/I-287 West toward Albany. Merge onto I-287 W/I-87 N/New York State Thruway North. Take exit 14A to merge onto Garden State Pkwy S toward New Jersey. Take exit 163 on the left to merge onto RT-17 S/N RT-17 toward Paramus/RT-4/G Washington Bridge. Continue to follow RT-17 South. Merge onto I-80 E via the ramp on the left to New York. Take exit 68A to merge onto I-95 S/New Jersey Turnpike S toward US-46/Meadowlands Sports Complex. Continue to follow I-95 South. Take exit 13 to merge onto I-278 E toward Verrazano Br./Goethals Br. Take exit 4 for Forest Ave. Turn left at Forest Ave. Turn right at South Ave. From Brooklyn and the East Take the Belt Pkwy West to exit 3 for I-278/Verrazano Bridge toward Staten Island. Keep left at the fork, follow signs for Lower Level and merge onto I-278 West. Take exit 6 toward South Ave. Merge onto Goethals Rd North. Turn left at South Ave.
