Thunderbird VA Health Care Clinic

Our Thunderbird VA Healthcare Clinic serves our Veterans from North/Central Phoenix, Glendale, Peoria, Scottsdale, Avondale, Sun City, Goodyear and Surprise. The clinic is based on the primary care-mental health integration model of health care that has physicians, nurse practitioners, psychiatrists, psychologists, social workers, dietitians, registered nurses and clinical pharmacists. The clinic does not have a pharmacy onsite, prescriptions will be filled by mail through the main facility. The clinic has a laboratory draw station and a part-time Veterans Service Officer. Appointments If you have a medical emergency, call 911. Medical visits are by appointment only. Your health care provider will coordinate the care you receive from other professionals on the team. If you are ill or have questions about your care, you are encouraged to call (602) 633-6900, or (602) 277-5551, ext. 1-4320. Please call to cancel any appointments you cannot keep. This allows us to schedule other patients who may be ill. To make your appointment more productive, we suggest that you arrive 30 minutes before your appointment and bring with you: All medications. Home blood pressure records. Diabetes diaries. All outside medical records including those from other VA facilities. Any other information that may apply to your care.
