Waterbury Outpatient Clinic

The Waterbury VA Primary Care Clinic provides general internal medical care. This includes care for immediate and chronic health concerns, preventive medical care, physical exams, diagnostic laboratory testing, general X-rays and limited immediate pharmacy services. ADVANTAGES Veterans will have one primary care provider to coordinate care on an ongoing basis. Veterans can receive primary medical care closer to home. Veterans will still be eligible for specialty outpatient care, inpatient care and all other entitlements through the West Haven or Newington Campuses. HOW TO ENROLL Please mail a completed application along with a copy of your DD-214 and discharge papers to: VA Connecticut Healthcare System 950 Campbell Avenue West Haven, CT 06517 Attention: Eligibility Office DIRECTIONS From I-84 West (from Hartford): Take the Union Street exit – Exit 22 – toward Downtown Waterbury. Turn left onto Union Street and then take the second right onto South Elm Street. At the first traffic light, turn left onto Scovill Street. The entrance to the Scovill Street parking garage will be on your right. From I-84 East (from Danbury): Take the Baldwin Street/Downtown Waterbury exit – Exit 22. At second light, turn left onto South Main Street. Proceed straight after the first traffic light and turn right onto Scovill Street. The entrance to the Scovill Street parking garage will be on your left. From Rt. 8 North (from Bridgeport): Merge onto I-84 East via Exit 31 (on the left) toward Hartford. Take the Baldwin Street/Downtown Waterbury exit – Exit 22. At second light, turn left onto South Main Street. Proceed straight after the first traffic light and turn right onto Scovill Street. The entrance to the Scovill Street parking garage will be on your left. From Rt. 8 South (from Torrington): Merge onto I-84 East via Exit 31 (on the left) toward Hartford. Take the Baldwin Street/Downtown Waterbury exit – Exit 22. At second light, turn left onto South Main Street. Proceed straight after the first traffic light and turn right onto Scovill Street. The entrance to the Scovill Street parking garage will be on your left.
