Description of Program: The Yuma Community Based Outpatient Clinic (CBOC) is designed to deliver Primary Care Services to eligible Veterans living in Pima, LaPaz and surrounding counties. Hours of Operation: Monday through Friday 8:00 a.m. – 4:30 p.m., closed all Federal Holidays Services and Scope of Care • Comprehensive Primary Care • Acute and Chronic Disease Management • Women’s Health Care • Specimen Collection Laboratory • Mental Health • Social Work Services • Eligibility and Enrollment • Medication Education and Counseling • X-Rays • Optometry Services • Nutrition Education and Counseling • Physical Therapy • Audiology • Returning Combat Veterans Program • Telemedicine/Telehealth Education • Health Screening, Promotion, and Education Eligibility: For information about enrollment in the Yuma Community Based Clinic call Eligibility and Enrollment (EEC) at (928) 317-9973.
