Services Available The Hollywood VA Community Based Outpatient Clinic is open from Monday through Friday, 8 a.m. to 4:30 p.m. It does not offer emergency services. The following services are available: Primary care Electrocardiogram (EKG) Laboratory (phlebotomy) Audiology Immunizations Each veteran will have his/her health care concerns handled by our primary care teams who will then become their personal medical provider. All patients are encouraged to call 954-986-1811 between 8 a.m. and 4:30 p.m. Monday through Friday, except holidays, if they have any health questions. These questions will be answered by the patient's primary care team. The pharmacy will provide pharmaceutical care for veterans who are seen at this clinic. Initial prescriptions for medications will be provided during the clinic visit. Medications for continuous therapy will be provided through the prescription mail out program. Requests for prescription refills will be mailed to Pharmacy Service, Miami VA Healthcare System, or through the automated telephone system. Medication refills or information about your VA prescribed medications can be obtained by calling the Miami VA Healthcare System Pharmacy at 305-575-3133 or 1-888-276-1785. Registration and Enrollment Information Pre-registration at the Miami VA Healthcare System or at this clinic is required. Walk-ins are accepted pending the availability of appointments. Patients not previously registered may do so at this location and will be given an appointment for a later date.

Driving Directions The Pembroke Pines VA Community Based Outpatient Clinic is located on Sheridan Street between State Road 7 (US 441) and University Drive. Services Available The clinic is open Monday - Friday from 8 a.m. - 4:30 p.m. It does not offer emergency services. The clinic provides the following services: Primary care Electrocardiogram (EKG) Laboratory (phlebotomy) Immunizations (influenza, pneumonia, tetanus and diphtheria) Audiology Psychiatry A service officer from Broward Veterans' Services is available once each week. Patients are encouraged to call 954-894-1668, between 8 a.m. - 4:30 p.m. Monday - Friday, except Federal holidays, if they have any health questions. There are no pharmacy services at this clinic. If patients have a refill, medications will be mailed to their home by the medication mail out service Registration and Enrollment Information Pre-registration at this clinic is required. Patients not previously registered may do so at this location and will be given an appointment for a later date.
