01 The Paradox

In 2014, a landmark study found that type 2 diabetics on metformin had lower mortality rates than non-diabetics not on metformin. This is paradoxical: diabetics should have higher mortality than non-diabetics. The finding was so surprising that it was initially dismissed as a statistical artifact. But multiple replication studies confirmed it. Metformin appears to have lifespan-extending effects independent of its glucose-lowering action.

02 The Mechanism

Metformin activates AMPK (AMP-activated protein kinase), the same energy-sensing pathway activated by exercise and caloric restriction. It also inhibits mTOR (the same target as rapamycin) and reduces inflammation. Essentially, metformin mimics the cellular effects of exercise and fasting. Unlike rapamycin, metformin has been used by tens of millions of people for decades, with a well-characterized safety profile. This makes it the ideal candidate for repurposing as an anti-aging drug.

03 The TAME Trial

The Targeting Aging with Metformin trial, approved by the FDA in 2025, is the first clinical trial with aging itself as the primary endpoint — not a specific disease. It will enroll 3,000 adults aged 65-79 and measure whether metformin delays the onset of age-related diseases as a group. If successful, it would establish aging as a treatable condition, opening the door for insurance coverage of anti-aging interventions. Results are expected by 2029.
