Chapel Street CBOC
Conveniently accessible by public transportation, Chapel Street Community Based Outreach Center (CBOC) is a hub for services related to homelessness, primary care, substance abuse and mental health. Breakfast and lunch are provided to homeless Veterans. The CRRC also offers on-site medical primary care and referrals to other VA specialty services as necessary. Chapel Street has an extensive network of community partners who can help with legal advice, vocational counseling, family care, spiritual care and information on benefits. They also work with the DHS to provide homeless veterans emergency housing with supportive services on site. VA Shuttle buses are available for patient transportation to VA’s NY and BK campus for referrals to other VA specialty services. The VA New York Harbor Healthcare System (VA NYHHS) consists of three campuses located in Bay Ridge, Brooklyn; the East Side of Manhattan; and in St. Albans, Queens. VA NYHHS has Ambulatory Care Centers at all three campuses plus Community Based Outpatient Clinics (CBOCs) in Harlem, downtown Brooklyn and Staten Island. The Brooklyn Campus is a tertiary care, academically-affiliated medical center located in Bay Ridge, Brooklyn. The Brooklyn Campus has bed services in acute medicine, surgery, psychiatry and residential substance abuse. Specialized programs exist in comprehensive cancer care and non-invasive cardiology. The Cancer Care Program includes special expertise in palliative care and radiation oncology. The Radiation Oncology Service is fully equipped with both state-of-the -art brachytherapy and teletherapy services. Three-dimensional radiotherapy, prostate brachytherapy (seed implants), and high dose rate (HDR) treatments are routinely available. The Brooklyn Campus also provides specialized cardiac care in the area of electrophysiology. AFFILIATIONS The Brooklyn Campus is affiliated with the State University of New York-Downstate (SUNY). A fully integrated residency program exists with SUNY in general medicine and specialties in cardiology, endocrinology, gastroenterology, hematology/oncology, pulmonary, nephrology, infectious disease and rheumatology; in general surgery and specialties of urology, orthopedic surgery, ophthalmology, and otolaryngology, anesthesiology; dermatology, pathology and rehabilitation medicine. The Optometry Service is affiliated with the SUNY School of Optometry. The medical center is affiliated with the New York University School of Dentistry. University level allied health training programs exist in nursing, audiology, speech pathology, occupational therapy, physical therapy, psychology, pharmacy and social work. Health Systems Specialist trainees and administrative residents and fellows regularly intern at the medical center. TYPE OF FACILITY Tertiary Care Medicine, Surgery, Psychiatry, Residential Substance Abuse SPECIAL PROGRAMS Veterans are enrolled in the medical center's primary care program, which establishes one healthcare provider (MD, PA, NP) and team, to coordinate the patient's care. A broad spectrum of inpatient and outpatient services are available in medicine, surgery, psychiatry, dermatology, rehabilitation medicine, pathology, nephrology, laboratory medicine and radiology. An innovative house staff rotation in ambulatory care medicine and primary care pharmacy makes the medical center a preferred site for training medical students. The medical center maintains a Comprehensive Cancer Care Center providing highly integrated oncologic, surgical and state-of-the-art radiotherapy services. VA NYHHS HIV/AIDS, Cardiac Surgery, Rehabilitation Medicine and Dialysis programs recently have been designated as VHA Programs of Excellence. Additionally, VA NYHHS has recently been approved for a Fisher House to be located on the Brooklyn Campus. A Women's Healthcare Center, with a dedicated mammography unit, exists offering comprehensive medical services to female veterans. Research and Development The Research and Development Service is an integrated program across VA NYHHS that is broad and varied in scope. It includes basic science research, clinical trials, and centers of excellence in most of the major programs offered by the VA Research Central Office such as co-operative studies, medial research, rehabilitation R&D and specialized centers. The major fields of investigation include, but are not limited to, cardiovascular electrophysiology, hematology/oncology, cancer, infectious diseases including AIDS, mental health and substance abuse, optometry and rehabilitation engineering. VA, NIH, AHA and other non-VA funding support the research activities. The total intramural funding is about $4.5 million and over $10 million at the affiliated medical schools and the VA NYHHS non-profit research corporation. Sharing Agreements The Brooklyn Campus has multiple sharing agreements with the Department of Defense (including TRICARE) and with various other federal agencies. Additional sharing agreements exist with the Metropolitan Medical Association, New York State Veterans Home at Montrose and the Salvation Army.
